STREET OUTREACH TEAM
|
The West London outreach service covers both Kensington & Chelsea and Hammersmith & Fulham. Based at the Broadway Centre, the multi-disciplinary team engage with both rough sleepers and the street population (generally clients who are housed however use the street community as their support network) and help them to receive quick and easy access to health care, drug, alcohol and mental health support and accommodation.
This is achieved through the following activities:
-
The team goes out on the streets early in the morning and late at night to identify and work with rough sleepers and in the day to develop relationships with the street population. By befriending and slowly building trust, the workers are able to assess the needs of the clients and refer them to appropriate accommodation and support.
-
Outreach workers work in partnership with the local drug and alcohol social services team and mental health team, identifying clients on the streets who would benefit from specialist intervention and support.
-
Part of the team provides tenancy support and are able to assist housed clients in managing any debt problems and access funding in order to improve the standard of their accommodation, therefore encouraging clients away from the streets.
-
The Outreach and Specialist Workers have set weekly times when they visit specific areas on the streets in order for clients to know when to seek advice and assistance when ready.
-
Provide a daily advice and referral service for rough sleepers. ( A weekly session is also provided at a Day Centre in Kensington & Chelsea).
-
The team encourage rough sleepers and the street population to access the centre where, in addition to housing support, they can access services to meet their basic needs like food, clothing, showers, primary health and social activities.

BROADWAY CENTRE
|
Winning the prestigious 2002 RIBA Design Award and Civic Trust 2002 Award, the Broadway Centre based in West London is a leader in its field providing a holistic and integrated service of support and care for rough sleepers and vulnerably housed people.
The Broadway Centre focuses upon the individual needs of each person who visits and empowers them to progress towards an independent lifestyle.
Every client who accesses the Centre is given a needs assessment and allocated a personal case worker where appropriate. Using a recognised assessment model, the case worker formulates a support plan to ensure that an individual's housing, mental, physical and social needs are being met, and that they are progressing towards positive outcomes. Project workers at the Centre provide support through individually tailored support plans created from the needs assessment.
The Centre also provides groupwork and life skill sessions to help increase peoples self confidence, self awareness and self esteem. Sessions also seek to develop personal and social skills and include arts and crafts, cooking and gardening. Individuals can access information, advice and support on a range of issues such as claiming benefits, debt management, tenancy problems, and gaining a referral into accommodation.
The Centre also offers a range of healthcare services, addressing the
health inequalities and barriers that clients face in obtaining care and treatment. Health facilities include an on-site GP service, nurse, podiatrist and optician. The Centre also has strong links with statutory health services. Through collaborative working practices staff are able to advocate and refer clients to drug and alcohol detoxification and treatment programmes.
Alternative health therapies such as ear acupuncture, indian head massage and shiatsu are offered as a less daunting way for clients to take steps to look after their health. Ear Acupuncture has proved very popular with clients with alcohol and drug dependencies, as it balances the body's energy system and reduces cravings; it also promotes relaxation and consistent sleep patterns.

SUPPORTED HOUSING NORTH, WEST
AND SOUTH LONDON |
Broadway’s Supported Housing Services division gives support to people living across 11 London Boroughs providing temporary support to single homeless clients living in shared and self-contained accommodation. The emphasis is to help promote independence and tenancy sustainment with clients who do not require 24 hour on-site support. The teams are based at four different sites over London which are Camden , Vauxhall, West Kensington and London Bridge .
Support is provided through the following processes:
The Assessment Process - Clients are referred into Broadway’s Supported Housing by street outreach teams, hostels and other homelessness agencies e.g. the Clearing House. Potential clients complete an assessment form, are interviewed by Broadway’s Assessment Team and have their needs carefully discussed by Broadway’s Assessment Panel, who make sure that the process of gaining a Broadway property is fair and equitable.
Once a client has been allocated a property, they are assigned a Housing Support Worker to provide them with the guidance and support they need to maintain their home. Housing Support Workers are trained to complete ongoing multiple needs assessments to ensure that the needs of each individual client are identified and addressed during their time spent with Broadway.
Support Plans - Housing Support Workers provide support through individually tailored Support Plans, created from the multiple needs assessment. Support Plans are developed and reviewed to identify issues such as financial, health and social barriers that need to be addressed for the client to develop the skills and confidence needed to successfully make the transition to independent living.
Keywork Sessions – Housing Support Workers arrange one to one meetings where clients can discuss their housing, mental, physical and social needs. The Support Worker uses keywork sessions to review individual Client Support Plans, and to monitor progress towards identified personal goals. Housing Support Workers provide practical and emotional support for clients through these sessions.
Health and Safety - The role of Housing Support Worker includes carrying out health and safety checks and dealing with maintenance issues at the properties we manage.

HARINGEY KEYSUPPORT
|
Broadway, in partnership with Community Housing Group, provides tenancy support to Haringey Council tenants.
A high proportion of rough sleepers at one time had a council tenancy but lost it arguably due to a lack of support. Many people struggle in council properties to sustain their tenancies.
This scheme is an opportunity for us to do much needed preventive work as well as build on our success in the Brent Floating Support Scheme in helping so called general need council tenants address their unmet housing related support needs. We also provide welfare benefits advice and help clients improve their literacy and basic skills.
